Job Description

Summary:

The Pediatric Psychiatry Resource Network (PediPRN) Rhode Island*s Child Psychiatric Access Program aims to improve access to behavioral health care for RI children by supporting pediatric primary care practitioners (PPCPs) and related child health providers.

Under the general supervision of the Clinical Director and Program Manager the primary role of the care coordinator is to provide resource and referral services to consulting practitioners and maintain a database of these services.

Additional responsibilities include community outreach gathering data preparing and maintaining data records triaging and scheduling consultation calls and coordinating/scheduling in-person evaluations when needed. Opportunities to participate in webinars and trainings may also be available.

Responsibilities:

Answer consultation calls and respond to online scheduled consultations. Support scheduling with PediPRN consultants.

Provide referral support:
a) to consulting practitioners and related staff regarding behavioral health and other community-based services for distribution to families;

b) directly to families (on occasion) to support addressing potential barriers to accessing behavioral health services

Referral support includes providing resources and recommendations for outpatient psychiatric treatment mental health groups specific supports the child/family might require for mental health and higher levels of care as needed.

Resource support includes providing tools for the practitioner to enhance their knowledge and/or to provide to the family as well as education and training opportunities.

Participate in outreach activities to support continued program enrollment and utilization and keep databases regarding available services up-to-date.

Continuously monitor research and curate appropriate resources to maintain/improve behavioral health resources/referrals database.

Provide feedback on systems and workflows to program manager and clinical director to enhance procedures.

Participate in program-related meetings including staff meetings community meetings relevant to resource development and resource distribution and program engagement.

Complete documentation within Lifechart according to established policies and procedures.

Administrative support related to program utilization outcome reporting and outreach.

Attend supervision with the PediPRN leadership.

Observe all hospital policies rules and regulations and all policies and procedures of the Outpatient Department.

Collaborate with other Lifespan services lines (Bradley Access Pediatric Emergency Services etc) to support development/maintenance and utilization of the resources/referrals database.

Collaborate with PediPRN team members to support them with consultations as requested.

Other duties as assigned by PediPRN Program Manager Clinical Director and/or Director and Practice Manager.

Other information:

A Bachelor*s degree in a related field may be considered when accompanied by at least one year of relevant experience in managed care activities.

Demonstrated knowledge and skills necessary to provide care patients with consideration of aging processes human development stages and cultural patterns in each step of the care process.

EXPERIENCE:

Relevant case management experience required.

Demonstrated ability to interact professionally both on the phone and with colleagues.
