Job Description

Summary:

To provide ongoing intensive assessment and case management to populations that are suffering with co-occurring disorders and behavioral health needs; outreach and crisis intervention to dually diagnosed (psychiatrically disabled/chemically dependent) clients according to their level of care. This shall include Community Psychiatric Supportive Treatment/Case Management (CPST) general medication maintenance substance abuse counseling individual family and group psychotherapy. Conduct substance abuse treatment with clients in need according to the 12 core functions of substance abuse counseling as defined by the ICRC.

Responsibilities:

Completes biopsychosocial evaluations. For persons assessed in need of substance abuse services complete the SUD section of the Biopsychosocial Assessment and include recommendations for a level and type of service based on current American Society of Addiction Medicine*s (ASAM) criteria.

Provides consultation regarding patient*s background psychosocial functioning and needs to medical and support staff in order to assist in planning a comprehensive care program for the patient.

Develops treatment plans and initiates clinical intervention utilizing individual and group treatment modalities and analyzes course and result of treatment. Assist the person to attain the goals of his or her person-centered treatment plan for recovery.

Educates staff regarding clinical intervention strategies related to symptomatology substance abuse sexual abuse and family and relationship issues.

Monitor Medication to ensure client understands prescribed dosage monitor side effects and is taken accordingly

Maintain caseload of clients assigned by Program Director/Supervisor

Coordinate activities make appointments and may include accompanying client(s) as noted on Treatment Plan

Overall objectives of services to ensure clients active participation in treatment maintenance of a stable residence in community development of ADL skills development of social support network and pursuit/obtainment of employment

Participate in daily staff organizational meetings and treatment planning review meetings with multidisciplinary team

Assists patient and families in understanding accepting and following medical and social work recommendations designed to restore the patient to optimum functioning within their capacity.

Assesses patient and family advocacy needs and coordinates appropriate advocacy activities within the hospital and in the community with an emphasis on self-determination for the client system.

Participates in at least one course seminar or conference for improvement of job skills per year. Maintains CEU requirements and maintains in good standing with LCDP licensure

Participates in direct contact with clients family and collateral contacts according to treatment goals. Minimum of 22 hours per week. with the primary focus on substance abuse interactions

As relevant to clinical assignment remains current on available community resources and accesses them as needed.

Maintains and enhances professional expertise through appropriate educational activities. May attend training for specific areas of clinical responsibility.

Provide services and interventions in accordance to grant funding programs.

QUALIFICATION REQUIREMENTS:

LCDP/or eligibility to test for LCDP upon hire Bachelors or Master*s degree preferred

CPR training within 90 days of employment and maintain annually

Maintain a valid RI license at all times and use of car during normal working hours or other means of transportation to complete major functions of job

Physical examination and Mantoux test annually

Must maintain automobile liability insurance coverage for vehicle used in accordance with center requirements


BASIC KNOWLEDGE:
Understanding of mental health and chemical dependency issues
Awareness of and compliance with HIPAA and 42 CFR Part 2

Analytical and therapy skills necessary to evaluate patients develops treatment plans initiates therapy and evaluates results. Ability to effectively interact with patients families medical staff and outside agencies in providing services

Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes human development stages and cultural patterns in each step of the care process.




EXPERIENCE:
At least 2 years of relevant experience working in the substance use disorder field or
one year of clinical experience preferably in a health or mental health setting with an emphasis on populations to be served in the assigned area.

Training and experience necessary to address the specific needs of the patients served in assigned areas of responsibility; this includes issues relating to age and stage of development and the particular special needs and cultural patterns of the patient populations being served.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Daily exposure to patient care with minimal normal office environment.

Ability to conduct patient care in the office community and patient home settings.
INDEPENDENT ACTION:

Performs independently within the department*s policies and practices. Refers specific complex problems to the supervisor when clarification of departmental policies and procedures are required.
