Job Description

Summary:
To provide comprehensive clinical services as part of a multi-disciplinary team assessment crisis intervention behavioral skill training case management education and counseling services to clients and their families t in order to assist clients in achieving treatment and rehabilitation goals. Coordinate and maintain accurate record keeping in accordance with all GHI Joint Commission funders and DCYF regulations.

Responsibilities:
MAIN DUTIES AND RESPONSIBILITIES:

Provide individual brief treatment and support to clients and their families in order to assist in developing coping skills for behavioral management and controlling psychiatric symptoms as prescribed in the treatment plan. Facilitate support group sessions as assigned.



Educate the team clients and families about mental illness and co-occurring substance abuse disorders and provide resources for community services as indicated.



Provide a minimum of 20 hours of direct service per week through case management outreach counseling assessment and intake activities.

Function as the primary worker for a caseload of clients as assigned by the Program Manager. This responsibility includes Intake assessments skills training resource linkage attending medication evaluation appointments and psychiatric evaluations treatment planning documentation of recovery and progress and any other duty as indicated by a comprehensive assessment of needs for each client and maintaining the clinical record with all necessary paperwork according to accreditation funding source agency and program standards and policies. Maintaining entitlement benefits for clients and insurance authorizations for services.



Assist implementation of discharge plans with both internal and external service providers including referrals coordination and communication of meetings telephone conversations correspondences and required documentation.



Act as the liaison with collateral service providers including insurers hospital staff community health providers primary care providers and other advocates.



Coordination and documentation of community intervention orders.



Establish maintain and close clinical records; including stocking forms preparing intake packets communicating with HID assuring confidential transport of records.



Maintain emergency and demographic data.



Following agency policies and standards for client rights and confidentiality.



Participate as a member of a multi-disciplinary team in the development and implementation of treatment plans. This includes but is not limited to: completing assessment assignments within time requirements specified by procedures scheduling/attending treatment planning meetings with staff and other individuals specified in procedures utilizes assessment information diagnostic results and staff reports in the development of treatment plans.



Provide support to milieu staff and direct client care as program require.



Attend biweekly supervision meetings weekly team meetings and other activities as required.



Maintain a valid driver*s license auto insurance and the use of a personal vehicle in order to conduct community home and hospital visits as well as to transport clients as needed.



Maintain annual CPR certification and any other related certifications / licenses as required or as needed.



Maintain all mandated agency training certifications as required. Complete a minimum of 5 hours of community education training to enhance knowledge of child and adolescent development family dynamics signs/symptom/impact of substance abuse or other related topics.



Maintain current knowledge of Agency P&P implement in daily routine and follow all procedures as indicated.



Serves on agency committees and task forces as appropriate and as assigned.



Other duties and responsibilities as assigned or needed.



CORE FUNCTIONS:

Establish and maintain protocols procedures and flow of clinical documentation within the ARTS program.



Provide services in the areas of assessment treatment planning consultation client education and referrals.

QUALIFICATION REQUIREMENTS:



NECESSARY SPECIAL REQUIREMENTS:




Maintain valid driver*s license at all times.





Ability to work evenings weekends and holidays on a rotating basis as needed.





Ability to communicate with clients and other staff face to face and by telephone.





Ability to obtain and maintain C.P.R. certification.





Ability to work as a multi disciplinary team member ability to accept establish and maintain working relationships with diverse populations of clients and staff ability to-maintain objectivity and ability to maintain perspective under stress; resilience.





JOB KNOWLEDGE SKILL AND ABILITY:



Ability to read analyze and interpret professional journals and governmental regulations.





Ability to write reports and business correspondence.





Ability to effectively present information and respond to questions from groups of managers clients customers and the general public.





Knowledge of and/or willingness to develop computer literacy skills.





Ability to successfully complete all required training and certifications





Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication.





Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals either manually or by use of a calculator.
