Clinical Denial Prevention The Clinical Pharmacist reads and interprets all medication orders and prescriptions received by the pharmacy. Additionally, the clinical pharmacist compounds and dispenses medications and supervises pharmacy technicians and interns who do the same. The clinical pharmacist will also assist with various clinical activities including rounding, attendance at CODE/Trauma cases, reviewing Clinical/Operational reports for appropriate, cost effective therapeutic interventions, and staffing pharmacy outpatient services including anticoagulation, diabetes and MTM clinics. The clinical pharmacist will also assist with precepting pharmacy students, educating nursing and/or medical staffs and assist with quality efforts through serving on and preparing materials for hospital committees/initiatives as requested.

Responsibilities

  • 1. Competent in the Practice of Pharmacy:
    • 1.1 Does not have high error rate when entering orders, making IV medications or checking Automated dispensing cabinet refills.
  • 2. Controlled Substances Inventory and Record Keeping
    • 2.1 Dispenses narcotics/controlled substances according to hospital policy.
  • 3. Sterile Product Preparation
    • 3.1 Complies with required clean room attire and cleaning procedures. In the event of a technician or intern not complying, immediate corrective action is taken.
  • 4. Professional Conduct and Leadership
    • 4.1 Develops effective working relationships with pharmacy staff, nursing staff, medical staff and administration. Maintains positive attitude with respect to all hospital and departmental goals and objectives.
  • 5. Supports Hospital Information Systems and Maintains Educational and Safety Compliance
    • 5.1 Acts as resource for nursing, pharmacy and other staff for issues related to Pharmacy Automation or the pharmacy information system. Assists with troubleshooting of Automation problems, particularly on off shifts
  • 6. Clinical Practice
    • 6.1 Documents and completes all interventions, maintaining an adequate number each month as determined by the Pharmacy Manager.
  • 7. Educational Initiatives and Presentations
    • 7.1 Takes an active role in the precepting of pharmacy students from the various schools of Pharmacy.
  • 8. Operational Excellence
    • 8.1 Supports hospital and departmental efforts to achieve and maintain accreditation with all applicable regulatory bodies including The Joint Commission and RI DOH.

Qualifications

EDUCATION

Doctor of Pharmacy Degree or Bachelors of Science in pharmacy from an accredited college of pharmacy.

EXPERIENCE

Two years experience as a hospital pharmacist preferred. Certification in one or more specialty areas such as Anticoagulation, Diabetes Management, Ambulatory care, MTM, pain managemet, Pharmacotherapeutics, Healthcare IT systems or Nutrition Support strongly encouraged. Completion of accredited residency program preferred.

LICENSURE

Membership in a professional organization preferred.
