Description

The Community Behavioral Health Clinician will build trusting and supportive relationships in order to support members with complex behavioral health needs to remain independent and stable in the least restrictive setting. This role will perform assessments targeted at specific interventions to support members in meeting individualized care goals. The Clinician facilitates member access to medical, behavioral and social services by playing a role as liaison or intermediary with medical, behavioral and social agencies. Works collaboratively with other care team members and any external provider agencies to ensure continuity and culturally competent, integrated community-based care.

Duties and Responsibilities:

Responsibilities include, but are not limited to, the following:

  • Must be comfortable completing visits independently and/or as a team in various community settings which may include, but not limited to, member homes, nursing/rest homes, group homes, shelters, provider offices.
  • Makes community and home visits to members to perform assessments on member's complex social and behavioral health needs.
  • Dependability and flexibility- able to shift priorities to manage complex behavioral health needs.
  • Completes hospital visits to support transitional care management process
  • Performs post-discharge assessments and interventions aligned with individualized member needs.
  • Support members in actively participating in their care plan interventions to achieve their short- and long-term goals.
  • Coordinates the provision of services/care to meet the member's social and health needs. Aids members, families and caregivers to access basic resources such as food, shelter, transportation.
  • Identifies potential medical or behavioral health concerns and collaborates with member's providers.
  • Collaborates with other health professionals, including contacting primary care provider offices, care management staff and community social service agencies on behalf of the member
  • Prepares and participates in team meetings
  • Maintains appropriate written and oral communication and documents patient encounters and updates the care management software system within established time frames
  • Maintains member confidentiality within legal and regulatory parameters
  • Other duties as assigned

Qualifications

Qualifications

Required:

  • Active, current, unrestricted RN, LMHC, LCDP, LICSW or LCSW license in RI in good standing(Consideration will be given to individuals without current RI license. Must provide documentation confirming application within 24 hours of offer)
  • Minimum of two (2)years experience working in community behavioral health
  • Demonstrated experience working with Medicare/Medicaid (MMP), Medicaid, Medicare or commercial products
  • PC based software programs intermediate level of proficiency
  • Strong organizational and interpersonal skills
  • Customer service orientation
  • Ability to work independently and prioritize activities
  • Must have access to reliable transportation to perform assessments in the community. If using personal vehicle, must have active driver's license and auto insurance. Frequency of travel: daily
  • Physical ability to perform assessments in the community

Preferred:

  • Bachelors degree
  • Masters degree
  • Bilingual
  • Substance Use Disorder expertise
  • Utilization Management or Post-discharge experience
